Obtaining an M.S. on the way to a Ph.D.

Here are the forms for a Mathematics Ph.D. student to use to pick up an M.S. along the way to a Ph.D.:

• MS Degree program form: Point to http://www.grad.umn.edu/current_students/forms/masters.html , then look under "Degree Progress:" and click on "Degree Program Form". This form can be filled out in a manner very similar to the Ph.D. degree program form.
• Examination Report Master's Degree and Certificate of Specialist in Education: This form is not available on-line, but is in the graduation packet, which can be ordered at http://www.grad.umn.edu/current_students/forms/grad_packet/masters/confirm.html. This form should be brought to the Ph.D. prelim oral, to be signed by the examiners.
• Application for degree:. This form is only signed by the student, and is turned in to the registrar. It is available in the graduation packet, which can be ordered at http://www.grad.umn.edu/current_students/forms/grad_packet/masters/confirm.html.
• (Optional) Commencement form: This is also available in the graduation packet, which can be ordered at http://www.grad.umn.edu/current_students/forms/grad_packet/masters/confirm.html.
